Referrals

 

North Shore Neurology accepts neurological & EMG referrals for patients aged 18 and over

 

Please fax referrals to: 604-960-2144

 

On the referral, please include:

• Patient's full legal name, birthdate, PHN and contact information

• Reason for consultation (detailed reason for urgency if requested)

• Referring physician's name, licence number and fax number

• EMG Referral form can be downloaded here

Once  the faxed referral is received, the medical office assistant will contact the patient directly to arrange an appointment with one of our neurologists.

 

Please also fax any relevant information such as medication lists, results of brain imaging etc, if available.
